Please be prepared with the following information:

  • Patient name and date of birth
  • Sending facility name and phone number
  • Sending provider name and phone number
  • Copy of patient chart
  • Diagnosis and clinical acuity
  • Requested service/treatment
  • CD/films of MRI, CT, echo and angio reports
  • Images, where applicable

Saint Francis is staffed 24/7 by clinicians trained in critical care, hospitalist medicine and emergency medicine with access to other subspecialties as needed.
